Notice Regarding Availability of Form 1095-C

In accordance with the Paperwork Burden Reduction Act, Kyanite Mining Corporation is no longer required to automatically provide Form 1095-C to each applicable individual.

However, Form 1095-C are still available upon request. If you would like to receive a copy of your Form 1095-C, you may request it by contacting Human Resources. Requested forms will be provided within 30 days of your request or by January 31, whichever is later. Kyanite Mining Corporation continues to meet all IRS reporting requirements, including filing Form 1095-C with the IRS, as well as complying with any applicable state requirements.
